Meet Your Team
Call us now at (877) 898-0685.
The Choices Team includes a physician, nurse practitioner, nurse, social worker and chaplain.
The Choices provider decides, along with the patient, who on the team will be involved in the care. For example, patients enrolled in the Choices program may not need to see a social worker or a chaplain, or the social worker may just make phone calls to the patient rather than visits. The makeup of the team and what the patient receives from Choices is based on their needs and how we can best meet those needs.
